Welcome to our Wildcat Family!

We're so glad you've chosen BrightStar Learning Center for your child's early years! Now that your child's preschool application has been accepted, we need the following forms completed before your child can attend.

All required forms must be properly completed and are due BEFORE August 23rd.

If enrolling late, please submit these forms on or before your child's first day of

preschool. Required forms must be completed by a parent or legal guardian

of an enrolled student. Online forms are signed electronically by entering your

legal first and last name and submitting the form. Only enter initials where indicated.

Incomplete forms will not be accepted.

Important note: These forms are due annually even if your child previously attended BSLC.

REQUIRED FOR ALL STUDENTS

*PLEASE NOTE: All parents are required to bring the child's original birth certificate and the immunization form provided by the child's pediatrician along with the forms above.
